ON CAMPUS Columbia County School District The Columbia County School Board will be accept- ing Out-Of-County Waiver request during the months of May and June, 2009. Applications are available on line at www.columbia.kl2. fl.us or at the School Board Office. The Florida Department of Education has released its 2009-2010 school entry requirements for students entering the district Prior to entry, attendance, or transfer to Florida schools, each child shall have on file a Florida Certification of Immunization, DH 680, documenting the following: Public, Non-public schools, kindergarten through grade 12 four of five doses of diphtheria-tetanus-pertus- sis (DTaP) vaccine; two or three doses of hepatitis B .vaccine; three or four doses of polio vaccine; two doses of measles-mumps-rubella (MMR) vaccine; two doses of varicella vaccine for kindergarten and grade one, and one dose of vari- cella vaccine for grades two through eight (the vaccine is not required if there is a history of varicella disease documented by the health- care provider); additional seventh grade requirement - in addition, children entering, attending or trans- ferring to the seventh grade in Florida schools must have an effective 2009-2010 tefanus-diphtheria-pertussis (Tdap) vaccine. Columbia City Elementary. School Chase Broome, a fifth grader at Columbia City Elementary won 1st place at the County Wide 4-H Tropicana Public Speaking Competition. She was awarded a plaque and a 4- H camp scholarship worth - $160.00. She then participate ed in the 4-H District Publid Speaking Competition in Bradford County and also . won first place. Eastside Elementary School Gale Miller's class has been studying all about Florida and having a blast doing it Students have been reseaching and studying Florida's history,- geography and ecosys- s terns.
