Haven Hospice of North Central Florida serves 16 counties. Six counties are in the Suwannee Valley region Hamilton, Suwannee, Lafayette, Columbia, Union and part of Baker. On average, Haven Hos- pice serves between 40 and 50 patients a year in Hamilton County. Haven Hospice has several programs to assist patients and their families during a critical illness. The Transitions Community Outreach Program provides support, edu- cation and community referrals for those dealing with chronic illness before they are eligible for or wish to use Haven Hospice, The Healing Hearts Program offers ongo- ing grief counseling and support to those faced with a loss, whether they have used other Haven Hospice services or not. The Pegasus Program is a pediatric program that gives emotional and spiritual care to children and their families that are experi- encing terminal illness and loss. The Ad- vanced Care Planning Program supplies information about advance directives and health care surrogacy.
